TOday, I finally submit the last two forms to complete the approval of medical leave to my company.
In these two forms, I referred your name, clinic address and the phone number in case.

I could be able to get all other procedures of medical leave approval with a help of Dr. Ahn Jae-hyun who is orthodontist in Bay area.
SInce medical leave is only possible to critical case, he diagnosed my illness and IMF(inter-maxillary FIxation), TMD (Temporomandibular Joint Disorder) that may cause joint malfunctioning.

I"d like to let you know in case there will be the phone call made to you.
I hardly believe you will get such a phone call though.
